Pathways to Family Support | Serving Halifax, Charlotte and Mecklenburg counties
TCCAA Infant and Maternal Health
Tri-County is currently collaborating with specific partners to develop and guide services within the service region of Southside VA (See list below).

Program Overview
TCCAA’s Infant and Maternal Health Services provide support and resources to new and expectant families from prenatal through postpartum and for families of young children up to three years old. Families are referred to our Head Start program, which can ultimately lead to participation in our Mentor Role Model Program for services.

IMH Services Outreach
This program provides community outreach and supportive programs to the community with an emphasis on expectant families, families with infants and children up to three years old.

IMH Home Visiting Services
IMH Home Visiting Services provides Community Health Workers to work one-on-one with a family who is expecting a baby or has a young child three years of age or younger. Supportive services may be provided in person at your home, by text, by phone, or virtually.
What can Infant and Maternal Health do for you?
Our services are designed to ensure that family goals are being achieved, which is why we provide ongoing case management to families receiving home visiting services. We also make referrals to other community resources that may assist with emergency and financial assistance, child support and paternity services, counseling, child care, adult education, employment training and job placement opportunities.
Program Enrollment
The Infant and Maternal Health program is free, voluntary, and can serve families until the child reaches the age of three years old. Enrollment is offered to all parents residing in Halifax, Charlotte, and Mecklenburg Counties.
How Services Are Delivered
Highly trained Family Support Workers provide home visits that include information on age- age-appropriate stimulation and play, to help develop bright and curious minds. We provide referrals to IMH Coalition partners, including routine development screenings, immunizations, wellness checks, nutrition, activities to create strong bodies, and how to care for sick children.
In-Home Visitation
We provide home visiting, focused on building positive parenting skills and healthy children. We help families with children up to age three, utilizing the Growing Great Kids Curriculum. We provide information about child development and discipline, learning new coping skills, and provide linkages to other community resources.
